Rich Text Formatting

Add rich text formatting options to the original task form request (text area / description fields) so bullets and bolding can be maintained if clients paste in copy from a doc.

Also, add bullets to the little text toolbar so people know you can add that formatting / easier to access. I happily stumbled upon them by using a dash that then turned into a bullet.
